Apply to Join OMIC
A separate application must be completed for each person or entity to be insured.
Create a new application to join OMIC:
- Ophthalmologist (use if you are in solo practice or you practice with a group not insured by OMIC)
- Group Ophthalmologist (use if you are joining an OMIC-insured group or your group is applying to OMIC)
- Medical Entity (the entity must provide predominantly eye care-related health services)
- Outpatient Surgical Facility (OSF) (must provide predominantly eye care-related surgery services and be owned by OMIC insureds)
- Medical Spa (OMIC must insure the medical spa’s owner)
- Optometrist (OMIC must insure the OD’s employer)
- Certified Registered Nurse Anesthetist (OMIC must insure the CRNA’s employer)
